Insurance

Anytime you have an appointment at Student Health, bring your insurance card! We will need to copy it for our records, and you will need to provide the policyholder's date of birth and employer.  We will verify this information at EACH visit! 

If you have charges after your SHS visit, you may pay by cash, check, charge to your U-Bill, or we will bill to your insurance company. We will submit charges to insurance companies with a U.S. mailing address, if we have signed authorization.  Click here to download and print off our insurance form. If you have questions about whether SHS charges will be covered by your insurance, contact your insurance company directly at their toll-free number (there is usually a 1-800 number on the back of your card.) To determine network coverage, use one of our provider names versus The University of Iowa or Student health Service.
