Insurance Card

Anytime you have an appointment at Student Health, bring your insurance card!  You will need to provide the policyholder's date of birth and employer.  We will verify this information at EACH visit!

If you are a new patient, or any time your insurance changes complete this insurance form. It also helps expedite check-in if you can send in a copy of the front and back of your insurance card in advance, whether it is your policy or your parents.  If you have SHIP or UI GradCare, submit your information via MyChart. You can check MyChart any time to see if we have your current insurance information.

If you have charges with your Student Health visit, you may pay by cash, check, or charge to your U-Bill.  We can submit charges to your insurance company with a U.S. mailing address.  Any charges not covered by insurance will be placed on your U-Bill.
